I hereby certify the following to be true: I understand that nothing on this website pretends to be a diagnosis; a diagnostic procedure; medical or allopathic treatment of my symptoms; a substitute for laboratory testing; a substitute for seeking emergency medical help; guaranteed to alleviate my symptoms. I undertake to continue all medications prescribed to me by my physician during my participation in WINKIAW and will contact my physician if I experience any new symptoms. I understand that research is currently being conducted on WINKIAW Therapy, and that in accordance with ethics of research, if details of my case are published in scientific or other journals, they will be altered and disguised to protect my identity. I also understand that WINKIAW Therapy is not covered by insurance. I personally accept responsibility for all payments connected with my therapy. I understand and agree with the terms outlined above and ask to participate in WINKIAW Therapy.
